D4 Medical – ONLY £ 50

You must undertake a medical assessment to obtain your provisional category C licence. A renewal HGV medical is required at 45 and subsequently every 5 years until it becomes an annual requirement from 65.

There can be exceptions to this rule though and you should always check the date in column 11 on the back of your driving licence.

For renewals , the DVLA will also send you renewal paperwork by post 56 days before your licence expiry.

A Group 2 license is required by HGV/LGV, PCV and Minibus drivers which includes the following vehicle categories:

Lorry
Bus / Coach
Ambulance (C1 Medical)
Minibus (D1 Medical)

ID Required
Please bring original photographic ID, your passport or drivers licence only.
D4 Medical Form
Download & Print your D4 Medical Form or Order it by post to your address.
Medical Summary
Please bring an upto date medical summary which can be obtained from your GP

Bring your driving glasses or contact lenses with your most recent prescription.
Medication List
Please bring your medication list/doses and medical conditions. A repeat prescription is preferred.
Arrive 15 minutes before
Please arrive 15 minutes before your appointment ( as there is a questionnaire to be filled in )

How do you obtain your Full Medical Record?

This is a document that tells us about your key clinical information, if any. You can obtain this at your local doctors surgery (GP) to print this off your full medical records, this should be a free service.
